For your convenience, we provide several online applications to assist you in your financial planning.

  • MEMBERSHIP APPLICATION

    Our Membership Application includes:

    • Debit Card
    • ATM Card
    • Home Banking
    • Mobile Banking

    Now, we offer two ways to join - online or print.

    New and existing may request a debit card in the Membership Application. Existing members can use the print option to apply for your PVFCU Debit Card:

    • Online: Complete the application online and submit securely to the credit union.
      – Membership Application
    • Print: Stop by one of our branches to complete the Membership Application
  • BUSINESS MEMBERSHIP APPLICATION

    Our Commercial Membership Applications includes:

    • Business Debit Card
    • ATM Card
    • Home Banking
    • Mobile Banking

    Complete Application, print, and submit with Required Documentation for Business Accounts, in person at the credit nion.

  • LOAN APPLICATION FORM

    Our Loan Application (Credit Card Application) includes:

    • Loans
    • Credit Cards

    Print: Stop by one of our branches to complete the Membership Application

    Online: New. Securely complete your application online and submit!

  • VOLUNTARY SKIP PAY FORM

    The Skip Pay request must be sent to:

    • Paradise Valley Federal Credit Union
    • via facsimile machine at (619) 475-0998
    • Or, mail to:
      Paradise Valley Federal Credit Union, 2700 E. 4th Street National City, CA 91950

    Regardless of the delivery method chosen, such request must be received by the credit union prior to the due date of the month’s payment you wish to skip, and will not be valid unless and until, your request is approved.

    Complete, print, and submit in person at the Credit Union.

    Complete our Change of Address form online.
